1. Please select one parent from your group of four to act as the group representative. 
 

2. Max 4 Skaters per group (you build your group with players at your same level of play – EX. house with house and travel with travel). Please sign your players up under their CURRENT level of play NOT what they will be this fall. 

 

3. The group representative will complete the intake form and submit the $400 group deposit. 
 

4. Once the form and deposit are received, PHS will reach out to confirm dates and times. 
 

5. Each family will then be booked by PHS individually through our online system and will receive confirmation for each skate. 
 

6. Each family will receive a separate payment link for the remaining balance of their skates. 
 

7. Each individual family must complete an e-waiver, unless one is already on file with PHS.
